• Once you’re in the sites page you will see a list of sites, when you have found your site, you will want to click ‘open’.




  • When you’re in your chosen site, you will want to scroll down and you will see the QR Code to report an Incident/Accident/Near Miss. You will then want to scan the QR Code.





  • Once you have scanned the QR Code it will bring you to this page here where you will need to fill out the information about the incident. Graphical user interface, application, Teams

  • Once you have filled in all the information you will want to scroll to the bottom where you will see there is a section where you can upload any files associated with the incident. Once you’re happy with your form, please click ‘Submit Report’. 




  • Once you have clicked ‘Submit Report’ it will pop up with a success message. This is confirmation that your form has been submitted. This will then send an email to all admin users notifying them that an incident has been logged to the system.


  • Admins will then need to go to the ‘Accidents’ section of Wise and scroll to the section below. Once the incident has been located, press the blue ‘Review’ button.




  • When you click ‘Review’ it will bring you to this page. The admin will then confirm the details of the incident.



  • If you scroll down to the bottom of the page there will be an ‘Approve’ and ‘Decline’ button. The admin has the ability to either approve or decline the incident form.






  • If you click ‘Approve’ it will pop up with the box below, you will then have two options on how you want to approve it. If the incident needs immediate action, you will select ‘Observation/Hazard Identification’. If you select ‘Accident/Incident/Near Miss’ it will put this entry into the accident log



  •  If you click ‘Observation/Hazard Identification’ it will appear with this pop up where you will need to fill in the information. The contents in the message box will be sent to the selected assignee via email. The proposed solution will then be linked to this finding once it has been submitted. Click ‘Save Changes’ after you’re done.





  • When you click ‘Save Changes’ it will bring you to the findings page where you will need to filter out the finding you just submitted and click ‘Review’ once you have found it




  • When you click ‘Review’ it will pop up with this screen here where you will want to enter the details of how you resolved the finding in the box, you can then add a photo if you have one, please note that the images are JPEG format only. Finally add a Close fault date in the bottom left and then click ‘Resolve Finding’.





  • On the other hand if you select ‘Accident/Incident/Near Miss’ it will bring you to the accidents page.






  • On the accidents page you will need to scroll down to the ‘Accident/Incident List’, and you will see your accident that you have submitted, and it will appear as a standard accident in the log.
