How to create a Risk Assessment on WISE

- Once you’re in the ‘Risk Assessments’ tab, you will see a list of Risk Assessments that have already been generated. To add a new Risk Assessment, you will want to click ‘Create New Risk Assessment’.
- Once you’re in the ‘Risk Assessments’ tab you will see at the top of the page are the filters, you will need to select a site from the list and choose a Re-assess date and if you have the Document Reference then enter that as well. After you have sorted your filters, you will see there is a list with all the activities you can choose from. If you click on an activity on the left side, it will bring it to the selected side on the right. Please bear in mind that you only have a maximum of 50 hazards per document. Once you have selected the number of activities you want, then click ‘Continue’.
- After you have clicked ‘Continue’ it will bring you to this page where it will give you the option to change the exposure frequency and exposure duration for the items you have chosen. Once you have adjusted your exposure frequency and duration you can click ‘Continue’.
- When you click ‘Continue’ it will bring you to this page here where you can make any necessary changes if needed. You will need to do this multiple times depending on how many items you selected. You can adjust the severity of the risk, likelihood of the risk occurring and the Risk Assessment. You can also remove a hazard if need be.
- Once you have made all your amendments, you will want to scroll down to the bottom and click ‘Continue’. When you click continue this pop up will appear asking for your signature, you will want to sign with your signature and add any further comments in the ‘Comments on Edit’ box. Once you have done this and are happy, you will want to click ‘Save’. Once you have clicked ‘Save’ it will bring you back to the main ‘Risk Assessments’ screen where you will be able to find the Risk Assessment you have just generated.
